We have adopted a Health System Data Framework with a central Personal Health Record focus to organise all our educational modules.  This is explained in further detail below the diagram.

The health informatics body of knowledge is defined within a personal health record (PHR) framework as PHRs collect, store, make available key data used for multiple purposes.

The diagram is three dimensional showing how personal health data are collected and used at various levels within a global health infrastructure by various entities for multiple purposes.

Health data stored in personal health and medical records consists of clinical and non-clinical data, summaries of episodes of care, a record of specific visits or activities and may include statements of exclusion. In addition such health data needs to be associated with demographic and identifier data and may be associated with specific research protocols. In addition these data may be used in screening checklists or health survey questionnaires. These data form the basis for health information and knowledge.

Individual (Personal) Health Records:   At the personal level health and clinical data are collected and used for managing one's own health and wellness status with the assistance or under the guidance of personal health care providers and carers. An individual health record is defined as follows:

'a repository of information regarding the health of a subject of care in computer processable form, stored and transmitted securely, and accessible by multiple authorised users. It has a commonly agreed logical information model which is independent of EHR systems. Its primary purpose is the support of continuing, efficient and quality integrated health care and it contains information which is retrospective, concurrent and prospective' (ISO TR 20514:2004 Health Informatics - EHR Definition, Scope & Context).

Personal Health care providers & carers: Any number of personal healthcare providers and carers use these data as the basis for communicating health status, planning and implementing diagnostic, treatment, continuing health management, general personal care and support needs between individuals and other health care and support service providers who collectively work as a care management team to meet specific health and wellness needs of an individual within their own unique family, community, environmental and social support infrastructure.

Organisational System Data Usage: Organisational providers use PHR data for the purpose of resource allocation, service scheduling, operational, financial, resource and workforce (HR) management as well as quality, patient safety, quality, practice evaluation and performance measurement.

National Systems Data Usage: Any national health system has various national organisations including any number of jurisdictions who make use of aggregate PHR data for the purpose of monitoring trends, funding, public health management, new knowledge discovery, health policy development, implementation and evaluation, various statistical collections, development and provision of best practice guidelines.

International Data Usage: Internationally these data are used for comparative purposes and for monitoring and managing epidemiological trends including contagious disease outbreaks
